1. Make sure to read the Disclosures regarding our products and services located at the bottom of the Apply page.
2. Once you have read these Disclosures, click in the box next to "I have read the account disclosures", and then click on the button labeled "Continue to Application".

3. The application will appear in a new window.
4. In this new window, select the product you wish to open and the ownership type. Once selected, click located at the lower right hand corner of the application window. Maximize your screen or use your scrollbar if you can't see this button.
5. Enter information pertaining to the account and the applicant(s). After completing each screen, click .

   6. Once you have completed your application, you may print it or request that we mail it to        you.

   7. Before mailing the application to Ascencia, please review it to make sure that it is signed       by all of the account owners, and that all required items in the Account Opening       Information Checklist have been included.

Once we receive your completed application, we will begin the process of opening your account. You will receive an email letting you know that your application is in processing.

When your account is established, we will send a confirmation email, as well as a "Welcome Kit" with instructions on how to access your account.
